Eligibility for Medicare
Certified by attending physician and
hospice
director to have limited life expectancy with a
poor prognosis (usually 6 months or less if
the disease runs its expected course)
Hospice is on-call twenty-four hours a
day,
seven days a week
Medical supplies, equipment and prescription
medications that are related to the terminal
diagnosis and which are consistent with
our formulary
No homebound or skilled care requirement
All other Medicare benefits waived
