How
do I get Home Health Care Services?
Patients can be referred to the VNA of Rhode Island from a
variety of sources including physicians, skilled nursing
facilities, hospitals, family, friends, social workers,
resident services coordinators, adult day care centers,
senior centers, or a self referral. Please note that a
physician must approve a plan of care before any home
health services can be provided.
